Practice Policy
1. Treatment Plan Validity
- Item numbers and fees listed in your treatment plan remain valid for 90 days from the date of issue.
- Overdue treatment plans will require a new consultation with your dentist to review and confirm proposed treatments and fees, which will be charged at our standard consultation rate.
2. Health Insurance and Dental Extras
- We accept all major health insurance providers including Bupa, Medibank Private, HCF, NIB and CBHS.
- Please check your health fund policy to confirm which portion of fees may be covered by your level of cover.
- We process HICAPS claims in-chair so you only pay any out-of-pocket gap on the day.
3. Balgowlah Family Dental Practice Reserves the Right to…
- Cancel or reschedule appointments subject to availability and clinical suitability.
- Charge a cancellation fee for missed or late-cancelled pre-booked appointments. Please give us at least 24 hours notice where possible.
- Amend the conditions or pricing of any current offer at our discretion.
